Bu \u00e7al\u0131\u015fmada, vilayetlerdeki hekim arz\u0131n\u0131n b\u00f6lgeleraras\u0131 da\u011f\u0131t\u0131m\u0131nda ele edilen sosyal fayda d\u00fczeylerinin Osmanl\u0131 ve modern T\u00fcrkiye aras\u0131nda ya\u015fanan uzun d\u00f6nemli de\u011fi\u015fimini konu al\u0131nm\u0131\u015ft\u0131r. Uygulamada kullan\u0131lan veriler Osmanl\u0131 \u0130mparatorlu\u011fu i\u00e7in 1897 tarihli ilk resmi istatistik y\u0131ll\u0131\u011f\u0131 ve T\u00fcrkiye i\u00e7in ise T\u00fcrkiye \u0130statistik Kurumu\u2019nun (T\u00dc\u0130K) web sitesinden al\u0131nm\u0131\u015ft\u0131r. \u00c7al\u0131\u015fmada uygulama y\u00f6ntemi olarak Atkinson e\u015fitsizlik endeksi yakla\u015f\u0131m\u0131 kullan\u0131lm\u0131\u015ft\u0131r. Hesaplanan Atkinson e\u015fitsizlik endeksi katsay\u0131lar\u0131, Osmanl\u0131 d\u00f6neminde doktor arz\u0131n\u0131n vilayetler aras\u0131 da\u011f\u0131l\u0131mlar\u0131n\u0131n vilayet n\u00fcfuslar\u0131na g\u00f6re ciddi d\u00fczeyde dengesiz oldu\u011funu g\u00f6stermektedir. Bunun anlam\u0131 ise n\u00fcfusun doktorlardan sa\u011flad\u0131klar\u0131 sosyal faydadan ciddi kay\u0131plar\u0131n\u0131n mevcut oldu\u011fudur. Osmanl\u0131 co\u011frafyas\u0131n\u0131n b\u00fct\u00fcn\u00fcnde n\u00fcfusun 1897\u2019de doktorlardan elde etti\u011fi sosyal fayda d\u00fczeyi %49.2, bug\u00fcnk\u00fc T\u00fcrkiye s\u0131n\u0131rlar\u0131 i\u00e7indeki n\u00fcfusun elde etti\u011fi sosyal fayda d\u00fczeyi ise %41.5 d\u00fczeyinde ger\u00e7ekle\u015fmi\u015ftir. Osmanl\u0131 d\u00f6neminin aksine, Cumhuriyet d\u00f6neminde kamusal sa\u011fl\u0131k hizmetlerinde ciddi iyile\u015fme ya\u015fanm\u0131\u015ft\u0131r. 2004\u2019te, n\u00fcfusun doktor arz\u0131n\u0131n iller aras\u0131 da\u011f\u0131l\u0131mlar\u0131ndan sa\u011flad\u0131klar\u0131 sosyal fayda d\u00fczeyler, uzman hekimlerden %64.3; pratisyen hekimlerden %84 ve t\u00fcm hekimlerden ise %77 oran\u0131ndad\u0131r. Dolay\u0131s\u0131yla uygulama sonu\u00e7lar\u0131, iki d\u00f6nem aras\u0131nda hekim b\u00f6lgeleraras\u0131 da\u011f\u0131t\u0131m\u0131ndan sa\u011flanan sosyal faydan\u0131n keskin \u015fekilde iyile\u015fti\u011fini g\u00f6stermektedir<\/span><\/span>.\u00a0<\/span><\/span>\u00a0<\/p>\n

Abstract<\/strong><\/span><\/span><\/p>\n<\/td>\n<\/tr>\n

This paper looks at long-term change of inter-regional human utility from medical doctors in provinces between Ottoman and Modern Turkey. Using data from the first statistical yearbook of the Ottoman Empire for dynastic period and TUIK web side for republic period and applying method is based on Atkinson inequality index. The indices have revealed that there is imbalance in the regional distribution of doctors when compared to the population in the 1897. It is mean that there are dramatic declines in the rate of social utility for the people. According to that have been calculated for people, level of social utility from pshycians had been found out to be 49.2% for all over the Empire and 41.5% for the provinces in the border of Turkey in the 1897. In contrary to the Ottomans period, there exist high levels of improve in social utility of health service in modern Turkey. Level of social utility from specialized pshycians have been found out to be 64.3%; from medical doctors 84, and from both of doctors 77.3% in the 2004. It is shown that the inter-regional social utility from medical doctors has improved between two periods.<\/span><\/span><\/span><\/p>\n<\/td>\n<\/tr>\n
